Long-Term Care

The decade-and-beyond view of transfeminine health. HRT effects stabilize in the first 2–5 years; everything after that is maintenance — keeping hormone levels in target range, screening for the cancers that retained anatomy still produces, monitoring bone density and cardiovascular markers, and managing the ordinary aging process on top of a body shaped by estrogen.

This section is the deliberate continuation of the medical pillar past the acute-titration phase. If you started HRT five days ago, the HRT section is your home; come back here at year three. If you've been on HRT for a decade and want to understand what to monitor, or if you're 50 and wondering how the next 30 years play out, this is the right place.

Audience

This section assumes 2+ years post-transition. New patients should start with HRT and the surgery sections; the long-term content here will keep until you've cleared the acute window.

How to use this section

  • If you're at year 2–5, start with The 5+ Year Trajectory and skim the screening pages so you know what monitoring should be in place.
  • If you've been stable for years, the screening and cardiovascular pages are the most consequential ongoing reads.
  • If you're 40+, Cardiovascular Health and the HRT-route-switch logic deserve focused attention.
  • If you're planning ahead, Elder Care and Fertility & Parenting Paths are the planning anchors.

What's not here

  • Specific medical advice. Every recommendation here is general; your individual screening and monitoring schedule belongs to your PCP and endocrinologist.
  • A substitute for primary care. This section helps you ask the right questions; it does not replace an actual care team.
  • Surgical revision detail. Late-life revision considerations are noted here; the procedure-specific detail lives in the surgery sections.
